About Hammersley Homes
Established in 2018, Hammersley Homes is a charity that provides support for life, for vulnerable adults who live with enduring mental health challenges and psychotic illnesses. Our aim is to establish a nationwide network of Day Centres, Outreach Services and Supported Housing schemes. We focus upon increasing people’s quality of life, whilst decreasing the likelihood of relapse into a mental health crisis. Implementing ongoing support provides an opportunity to notice if there is any notable decline in the person’s health or wellbeing.
About the Outreach Service
This service was set up to support individuals with long term mental illness who may be suffering from the debilitating effects of chaotic lifestyles, loneliness and/or social isolation. Our team of caring staff and volunteers provide companionship and friendship via home visits, remote telephone calls, our wellbeing drop in’s and our Pen Pal project.
Hammersley Homes supports each client with company and practical and emotional guidance, presenting options for each client to make informed choices. The Outreach Service encourages:

Main purpose of this role
The Outreach Programme Manager will manage the Outreach Home Visitors and Volunteer Outreach Home Visitors, providing ongoing support and guidance to ensure that appropriate and professional support is provided to our clients. The Outreach Programme Manager will liaise with referrers and work collaboratively within the local community areas in which we support clients, to develop and maintain a good working knowledge of the resources available. This is a fixed-term position for 1 year, with the possibility of extension subject to funding.
Principle duties
This role will involve:
- Management of the Hammersley Homes’ Outreach Programme, developing Outreach Services including group drop-ins and activities for our Outreach Members, expanding the our reach and increasing awareness of Hammersley Homes.
- Recruitment and induction of Outreach Home Visitors, both paid staff and volunteers.
- Support and management of the paid Outreach Home Visitors, through regular communication which will include email and telephone updates, monthly 1:1 sessions and wider team meetings.
- Collaborative working with the Admin Assistant, to recruit and induct Volunteer Outreach Home Visitors.
- Supervision of Outreach Volunteers through regular 1:1 meetings, as well as guidance and supervision of the paid Outreach Home Visitors as they supervise their Outreach Volunteers locally.
- Ongoing training of the Outreach Home Visiting team.
- Preparation of weekly rotas for each Outreach Home Visitor.
- Ensuring that the mileage / expense claims of Outreach Home Visiting teams are submitted monthly in accordance with the current Expenses policy.
- Providing leadership to the Outreach Home Visiting team on complex issues and casework.
- Coordination and delivery of internal Outreach team meetings and presentations to external organisations including Community Mental Health Teams and potential funders.
- Liaison with the local Community Mental Health Teams, to accept referrals, flag concerns and generally promote the professional development of the Outreach Service.
- Processing client referrals and completion of initial visits, ensuring the completion of person–centred support plans and relevant risk assessments.
- Completion of relevant risk assessments for activities involving clients using the Outreach Support Service, including PEEPS and Event Risk Assessments. (This task may be delegated to Outreach Home Visitors when appropriate training has been completed.)
- Overall management of the Pen Pal project, supporting the designated Pen Pal Volunteer, as required to monitor Pen Pal communications.
- Ensuring that all client records are kept securely and that confidentiality is maintained appropriately.
- Working collaboratively with the Hammersley Homes team and within the community to design and create engaging training and promotional materials.
- Working with the CEO and Fundraising Team to complete funding applications.
- Writing impact reports, when requested.
- Working collaboratively with the CEO to draft, update, and implement policies and procedures of relevance to the Outreach Service specifically and Hammersley Homes generally.
- Engagement in continuous learning, reflective practice and research appropriate to the Outreach Programme Manager role, ensuring that an up-to-date working knowledge of the sector is developed and maintained.
- Completion of the mandatory training courses detailed on the Training tracker and, in agreement with the CEO, any additional courses relevant to the role of Outreach Programme Manager.
- Monitoring and evaluating individual and service performance, in line with the aims and objectives of Hammersley Homes.
- Engaging proactively in monthly 1:1 meetings with the CEO and in Management level meetings with the CEO / Finance Officer.
- Acting as the Designated Safeguarding Lead for the Service, ensuring that Safeguarding concerns are reported correctly and dealt with professionally.
- Adherence to the policies and processes in place at Hammersley Homes.
- Any other duties, reasonably requested by the CEO, which are commensurate with the role.
Person Specification
Qualifications/Experience
1. A degree level qualification.
2. Previous experience of supporting vulnerable individuals.
3. A driving licence and use of a car for business purposes.
4. Experience of line management.
